All fields * are required. Only a single PDF file is accepted per document type. Example: Upload a single file, plans.pdf, that contains ALL pages of your plans, not page1.pdf, page2.pdf....
Any attempt to upload a document AFTER it has already been uploaded will overwrite the previous document.
